Person-Centered Care

To combat both caregiver burnout and ensure that elderly middle-class families have access to needed community-based long-term care services, Legacy Health Endowment has launched a small program in two rural California counties, Northern Merced County and Southern Stanislaus, Person-Centered Care (PCC). PCC is created to assist the patient and their family or caregiver in achieving their maximum level of independence while improving overall health outcomes. We are specifically targeting families that do not qualify for Medi-Cal (while we will likely include some Medi-Cal recipients) the goal is to focus on elderly and disabled adults who do not qualify for Medi-Cal.
