About the program
Why Women's Health 360 Was Created
Too many women in our community go without consistent, coordinated healthcare—often delaying care until issues become more serious.
Women's Health 360 was created to change that.
This program brings together preventive care, specialized services, and ongoing support into one coordinated approach—so women can prioritize their health without the burden of cost.
Funded by Legacy Health Endowment, this initiative is designed to expand access to care and support better health outcomes for women across our region.

Routine care to help you stay healthy and catch issues early
Heart Health Screenings
Proactive screenings to monitor and protect your cardiovascular health.
Breast & Cervical Screenings
Mammograms and Pap tests to support early detection and peace of mind.
Hormone & Menopause Support
Expert care for perimenopause, menopause, and hormone replacement therapy.
Mental Health Services
Support for anxiety, stress, life transitions, and overall emotional well-being.
Low-Cost Prescriptions
Access to medications for just $2, including hormone therapy.
Diagnostic Support
Up to $250 toward necessary diagnostic testing when additional care is needed.

Who It's For
Women’s Health 360 is designed for women who:
- Are 30 years or older
- Are not enrolled in Medi-Cal, Medicare, or TRICARE
- Live in one of LHE's 19 qualifying ZIP codes: 95301, 95303, 95307, 95313, 95315, 95316, 95322, 95324, 95326, 95328, 95334, 95358, 95360, 95363, 95374, 95380-82, 95388
Eligibility will be confirmed during the application process. Additional program requirements may apply.
