Person-Centered Care

To combat both caregiver burnout and ensure that elderly middle-income families have access to needed community-based long-term care services, Legacy Health Endowment, in collaboration with the City of Turlock, has launched the Person-Centered Care (PCC) program. PCC is created to assist the patient and their family or caregiver in achieving their maximum level of independence while improving overall health outcomes. The goal is to focus on elderly and disabled adults who do not qualify for Medi-Cal.
